When donating a car it is necessary to transfer ownership. Various states have different regulations on how this is done. This page is a collection of actual references from various state web sites.

1. How do I change my name on my Certificate of Title?
To change your name on your Certificate of Title, you can complete a Name Change /Correction Vehicle Ownership form MV2014 (36 KB). Return the completed form with your current Certificate of Title. There 's no charge for a name change.
You can also obtain form MV2014 at a DMV service center. If you would like the form mailed to you, send your request (include your mailing address) to the Bureau of Vehicle Services at bur-veh-services.dmv@dot.state.wi.us.
If you e-mail us about a specific vehicle, we will not be able to respond to your inquiry unless you provide us with the license plate number or the vehicle identification number.
2. How do I remove the lien from my title?
You will need to send in:
- Your original Wisconsin title (a photocopy is not acceptable) and
- A lien release card "Confirmation of Security Interest" which the lending institution should send to you
Or an original letter (copy or fax to customer is not acceptable) from your lender on their letterhead describing the vehicle by year, make and vehicle identification number, with a signature and position of the person signing.
Or the lender stamps your title with a “PAID” stamp which has the name of the financial institution, and signature and title of the authorized employee
There is no fee to remove a lien from a Wisconsin title. Send your Wisconsin title and lien release card or letter to:

If you can't locate your title, you may apply for a replacement title at the same time you have the lien cleared. The replacement title costs $8. (Note: Unlike some other states, Wisconsin sends the original title to the vehicle owner. It is not sent to the lien holder or kept by DMV until the lien is paid).
You may bring the application and lien release to any DMV service center offering title and registration services for immediate processing. There is an additional $5 fee for processing at the customer service center.
3. How do I apply for a duplicate or replacement title?
Application
If your Wisconsin certificate of title has been lost, destroyed, or stolen, you may obtain a replacement by filing an application for title - registration form MV1 (150 KB) or MV5 (234 KB) .
What is needed
The application must contain the vehicle identification number, the owner 's name and address, and the signature of the registered owner. The vehicle identification number can be found on your registration card, or on the vehicle itself. Only the owner shown in Division of Motor Vehicles (DMV) records may apply for a replacement title.
Payment
The application can be mailed to the address given on the application form. The fee for a replacement title is $8. You can receive priority service by including an additional $4 and mailing the application to the priority service address listed on the MV1 or MV5 application form.
You may bring the application to any DMV service center offering title and registration services for immediate processing. You will need to show proof of identity in the form of a driver license or identification card when applying for a replacement title. There is an additional $5 fee for processing at the customer service center.
Permission to pickup title is required
DMV now requires anyone picking up a replacement title for a vehicle they do not own, to present a completed "Permission to Pick Up Title" form MV2932 (79 KB) , and show proof of identity in the form of a valid driver license or identification card. This form is in addition to the MV1-Application for Title that should be completed in the owner 's name. If the requestor does not have written permission or proper identification, WisDOT will mail the title to the vehicle owner.
A replacement title voids all previous titles.

4. How is a vehicle transferred to a surviving spouse?
A surviving spouse can transfer ownership of up to five vehicles solely owned by his/her deceased spouse. The vehicle(s) may be titled in the name of the surviving spouse as owner or he/she may sign as surviving spouse to sell the vehicle. The transfer of ownership will not affect any prior liens upon the vehicle or vehicles.
When more than five vehicles that are less than 20 years old were solely owned by the deceased, the remaining vehicles must be disposed of through an:
- administrator
- executor
- court order
A copy of the death certificate is not required when the deceased owner was a Wisconsin resident and death occurred in Wisconsin. (Death certificate may be required for residents of other states.)

5. How is a vehicle transferred to an heir?
An heir or heirs may transfer ownership of a vehicle when a decedent leaves solely-owned property not exceeding $20,000 in value. An heir may title the vehicle in his/her name as owner or may sign as heir to sell the vehicle. In both cases, form MV2300 is required as a supporting document. A copy of the death certificate is not required when deceased owner was a Wisconsin resident and death occurred in Wisconsin. A death certificate may be required for residents of other states.
