1. How do I apply for a duplicate Title?
If you have lost or misplaced your title, you can apply for a duplicate certificate of title. Duplicate titles can only be applied for in the Title Office, Monday through Friday at the Pawtucket DMV, located at 100 Main Street, Pawtucket. You must fill out the Duplicate Title form completely, have the form notarized and pay a $25.00 fee.
· Only the owner(s) listed on the original title may apply for a duplicate title. In cases where the lien is still in effect, the lien holder may apply. This process can take up to 15 days.
· The duplicate title must be mailed to either the lien holder or the owner.
· If the original title listed a lien holder and the loan has been paid in full, a Release of Lien must accompany the application for a duplicate title.
· Owner 's signatures must be notarized. The notary signing the application must sign and print his or her name.
2. Salvage Title Information:
A vehicle that is considered 'salvage ' is one that has been damaged, such as in an accident, and deemed salvage by an insurance company. These vehicles can be repaired and re-sold in certain circumstances. Salvage titles are usually printed in red ink to distinguish them from a regular title.
· Red Rhode Island salvage certificates stamped "Class B - Repairable" must be accompanied by a Salvage TR-5 (VIN Inspection form) issued by the Enforcement section of the Rhode Island DMV.
· Red Rhode Island salvage certificates stamped 'Class A - parts only ' cannot be registered.
· Out of state titles stamped 'reconstructed ' or 'rebuilt ' can be registered with a police Salvage TR-5 (VIN Inspection form). The Rhode Island title will contain the words 'Reconstructed Salvage '.
· Out of state titles stamped 'salvage ' must have a Salvage TR-5 issued by the Enforcement section of the Rhode Island DMV in order to be registered in Rhode Island.
· The Rhode Island title issued will be stamped 'Reconstructed Salvage ' to indicate the vehicle was once a total loss but is now repaired, and this legend will be stamped on all future titles.
3. What do I need to do if I have changed my name?
You must change your name on your license, your registration, and your title.
- To change your name on your license, you must first have your name changed with the Social Security Administration. You should then go to one of the DMV locations with your new Social Security card and legal document showing the new name and the DMV will present you with a new license. There is a fee for this service.
- To change your name on your registration, you must go to one of the DMV locations with your new license, (the temporary license with your new name will suffice) your title, your legal document, and proof of insurance. You will be asked to fill out the TR-1 form and the cost will be $26.50. This cost covers the name change on both your registration and your title.
- To change your name on your title: if the title is in your possession, sign the title over to yourself by signing your former name on the back of the title as "seller". Then sign your new name as "buyer".
If the title is not in your possession, call the title office.
4. I need to title and register my boat/recreational vehicle. Where do I go?
The DMV does not provide this service. In order to register a boat, personal watercraft, or snowmobile please contact the Department of Environmental Management at (401) 222-3070.
5. I've moved and need to change my address with the DMV. What forms do I need?
The DMV has a change of address form available in any branch. You can also request that the forms be mailed to you by writing to the Pawtucket DMV. In addtion, that form can be downloaded from this website. On this form you must list your plate number AND your license number. Upon reciept of the request, the DMV will change your address information in its computer system.
