1. How do I obtain a duplicate title?
You must complete the 'Application for Duplicate Certificate of Title by Owner ' (Form MV-38O). If you are the lien holder of the vehicle, you will need ‘Application for Certificate of Title by lien holder ' (Form MV-38L).
Application forms are available online for downloading and at any messenger service office or any Driver and Vehicle Service Center location.

If your name has changed due to marriage, divorce or another reason, you will need to complete form MV-41A.
Side A of the form should be completed if you are only requesting an updated vehicle registration card. Side B should be completed if you are requesting an updated vehicle title.
